Student Physical Exams Required by Oct. 1

Students entering grades K, 2, 4, 7 and 10 are required to have their exams.

From the Lakeland Central School District website: 

New York State Law requires a physical examination for all students entering, Grade K, 2, 4, 7, 10 and all students transferring into the Lakeland Central School District. Physicals dated after September 1, 2011 will be accepted.

Physical forms or documentation of an up coming appointment should be returned by October 1, 2012 to the Health Office. Please make a copy for your records.

Students who do not return evidence of a physical examination or an upcoming appointment  will have an exam scheduled with our school doctor. Click here to download the form Health Certificate/Appraisal (Physical Examination).

A law recently enacted expands health screening to include the dental health of students in New York State. The district is requesting a dental certificate to be filled out by your dentist for students in Grades K, 2, 4, 7 and 10. Again dental exams are a requested not a requirement. Dental forms can be downloaded from the Districts web site. Click here to download the form Dental Health Certificate.
